ELIGIBILITY REQUIREMENTS
If you answer yes to any of the following, you may proceed to Step 1
You are applying for a United States Passport for the 1st time
Your expired United States Passport is not in your possession
Your previous United States Passport has expired and was issued more than 15 years ago
Your previous United States Passport was issued when you were under 16 and you are now over 16
STEP 1
GATHERING YOUR DOCUMENTS
A. Prepare Online DS-11 Application
Print Online DS-11 Application
Please complete the form, then press the "create form" button at the bottom of the screen. This will create a bar-coded DS-11 Application form. If your form came out correctly, it will have 3 bar codes: one on the left and one at the bottom of the 1st page, and one at the bottom of the 2nd page
B. Certified United States Birth Certificate or Naturalization Certificate
An acceptable United States Birth Certificate can ONLY be issued by the Office of Vital records or Vital Statistics from the State where you were born. The Birth Certificate must have a embossed or raised seal with a filing number. The Naturalization Certificate must be the original certificate. Copies of these documents ARE NOT ACCEPTED.
Your Birth Certificate has a registrars raised, impressed, embossed or multicolor seal, the registrars signature and the date the certificate was filed. It must be within 1 year of your birthdate.
Hospital Birth Records ARE NOT ACCEPTED. Please call the Office of Vital Check at 1-800-225-2414 for help with obtaining an expedited certified Birth Certificate.
If you DO NOT have a previous United States Passpot or Birth Certificate, you will need:
Letter of No Record
A Letter of No Record is issued by State Department with your name, date of birth, years that were searched for a birth record and that there is NO Birth Certificate on file for you. Make sure to supply as many of the following documents as possible:
- hospital birth certificate
- census record
- baptism certificate
- early aged school record
- doctors record of post-natal care
C. Proof of Travel
You must supply Proof of Travel when you go to an Acceptance Agent. It can be found in the form of a copy of airline tickets, travel itinerary or a copy of a company letter stating the name of the applicant and the travel dates.
If you fail to include proof of travel, your application will be placed on suspension by the Department of State Passport Services Agency
D. 2 Passport Photographs
You must obtain 2 Passport Photographs from an authorized Passport Photographer. You can get these photos at your local post office, drug store (CVS, Rite Aid, Walgreens), or UPS Store. They must be measured at 2 in X 2 in with a white or light colored background.
Here are some requirements:
- 2 x 2 inches in size
- identical
- full face, frontal view with a plain white/light colored background
- taken within the past 6 months, showing current appearence
- between 1 inch and 1 3/8 inch from the bottom of the chin to the top of the head
- taken in normal street clothes, uniforms shouldn't be worn in the photo
- no hats or headgear that blocks the hair or hairline unless worn for religious reasons
- if you wear prescription glasses, hearing aid, wig or similar articles, make sure to wear them for the picture

E. 2 Letters of Authorization
The Letter of Authorization authorizes ACE Passport Service to act on your behalf to discuss your application and case with the United States Passport Services Administration.
One copy of the letter will be sealed with your application for the Passport Services Agency, and the other MUST BE placed on the OUTSIDE of the sealed envelope for our records.
Failure to include a Letter of Authorization will place your passport on suspension

F. United States Government Fee
An application fee of $135.00 for ADULTS or $120.00 for CHILDREN made payable to: U.S. Passport Services and a $25.00 execution fee made payable to the Acceptance Agent at either your local Post Office or County Court House.

STEP 2
VISITING WITH AN ACCEPTANCE AGENT
You must visit an Acceptance Agent at your local Post Office or County Court House. Most people use their local Post Office. They are designated to handle the sealing of your DS-11 Passport Application, supporting documents and Letter of Authorization.
While at the Acceptance Agent, you will request that your DS-11 Form and supporting documents get sealed into an envelope and returned to you so that ACE Passport Service can expedite your passport. The sealed envelope can only be opened by an agent at the United States Passport Agency in Philadelphia, PA.
Make sure to include all of the items listed in STEP 1 when you visit with the Acceptance Agent. All items must be sealed. Also, provide a valid photo ID to prove that you are the person applying.

STEP 3
DELIVERYING YOUR SEALED ENVELOPE
Please send us your documents using a carrier that can track your documents. Please make a note of your tracking number in the case of a delay. Some commonly used carrieres are UPS, USPS, FedEx and DHL.
